115學年度國立暨南國際大學新生入學健康檢查通知

Notice of Health Examination for Newly Enrolled Students, National Chi Nan University, Academic Year 115

一、目的：為有效控制校園傳染病及落實全校學生健康管理工作，依規定新生入學一律須繳交健康檢查報告。
To effectively control communicable diseases on campus and implement student health management, all newly enrolled students are required to submit a health examination report upon enrollment.

二、依據︰教育部頒定之學校衛生法第8條:「學校應建立學生健康管理制度，定期辦理學生健康檢查」及學校健康檢查實施辦法之第4條:「學校辦理新生入學時，應進行學生健康基本資料調查，並做成紀錄。」故為維護學生健康，早期發現健康問題、早期治療，並預防傳染病，本校新生入學時依上述規定應完成體檢。

According to Article 8 of the School Health Act promulgated by the Ministry of Education: “Schools shall establish a student health management system and conduct regular student health examinations,” and Article 4 of the Regulations Governing Student Health Examinations: “Schools shall conduct a basic health information survey of newly enrolled students and keep records thereof.”

三、實施對象：

1.大學部一年級新生於115年9月2日上午統一辦理體檢。

(1) First-year undergraduate students shall attend the on-campus health examination on the morning of September 2, 2026.

2.因健檢量能有限，碩/博士班新生、在職專班、轉學生、復學生及直升研究所學生，請自行至醫院檢查，並於115年9月30日前繳交報告。
(2) Due to limited health checkup capacity, new master’s and doctoral students, in-service program students, transfer students, readmitted students, and students advancing directly to graduate school are requested to undergo the health examination at a hospital on their own and submit the report by September 30, 2026.

四、檢查時間：大學部一年級校內健康檢查時間：

五、檢查地點︰台中榮民總醫院埔里分院暨大門診。
NCNU Outpatient Clinic, Taichung Veterans General Hospital Puli Branch.

六、檢查費用︰為了確保健康檢查的品質，並減少您自行到醫院做體檢的奔波及花費，團體優惠價1100元整，健檢當天繳交給醫院。

To ensure the quality of the health examination and reduce the inconvenience and expense of arranging an individual hospital visit, the group discount fee is NT$1,100,to be paid to the hospital on the day of the check up.

七、檢查項目︰包括一般理學及牙科檢查、胸部X光、B型肝炎表面抗原及抗體、尿液檢查、血液常規檢查、血糖、尿酸、總膽固醇、高密度脂蛋白膽固醇、三酸甘油酯、腎功能、肝功能等共39項檢查。
The examination includes a total of 39 items, including general physical examination, dental examination, chest X-ray, hepatitis B surface antigen and antibody tests, urinalysis, complete blood count, blood glucose, uric acid, total cholesterol, high-density lipoprotein cholesterol (HDL cholesterol), triglycerides, kidney function tests, liver function tests, and other related items.

八、檢查須知：

(一）115年9月2日統一辦理大學部一年級新生體檢:
Unified Health Examination for First-Year Undergraduate Students on September 2, 2026.

1.請自行填妥健康檢查表正反面資料，並攜帶1吋或2吋照片（請勿黏貼於健檢表）、身分證或健保卡（外籍人士請攜帶護照或居留證），並於當天務必攜帶至現場。

Please complete both sides of the health examination form in advance and bring one recent photo (1-inch or 2-inch) and your National ID Card or National Health Insurance Card on the examination day. Foreign students shall bring their passport or Alien Resident Certificate (ARC).

2.填寫健康資料卡路徑：https://sso.ncnu.edu.tw/login-->登入新世代校務系統-->個人資訊-->填寫健康資料卡(不開放線上直接修改)

Please complete the Health Information Card via the University Administrative System:https://sso.ncnu.edu.tw/login→ Log in to the New Generation Administrative System→ Personal Information→ Fill out the Health Information Card

3.檢查前3日勿熬夜，需空腹8小時；請穿著無鈕釦、無金屬、無亮片的上衣，疑似懷孕者請勿照X光。

Please avoid staying up late for three days prior to the examination and fast for 8 hours before the examination. Wear clothing without buttons, metal parts, or sequins. Students who may be pregnant must not undergo the chest X-ray examination.

4.尿液檢查：用塑膠杯截取中段尿，再倒入尿液試管。如遇生理期仍需做尿液檢查，繳交檢體時請告知工作人員另做註記。

For the urine test, please collect a midstream urine sample in a plastic cup and transfer it into the urine test tube. Students who are menstruating are still required to submit a urine sample; please inform the staff when submitting the specimen so that it can be noted.

5.視力檢查：有配戴眼鏡者，請攜帶眼鏡以便測量矯正後視力。 

For the vision test, students who wear glasses should bring them for the measurement of corrected vision.

(二) 碩、博班新生、在職專班、復學生、非本校轉入之轉學生，請自行至公、私立醫院檢查：

Master’s/Doctoral Students, In-Service Program Students, Returning Students, and Transfer Students from Outside NCNU

1.同學亦可持【本校檢查表】至醫院辦理體檢，檢查費用須視各醫院收費標準而定，檢查後再將該檢查表繳回學務處衛保組。

Students may bring the NCNU Health Examination Form to a public or private hospital for the examination. The fee shall be subject to the standards of the respective hospital. After the examination, the completed form must be submitted to the Student Health Services Section, Office of Student Affairs.

2.校可受理最近3個月內其他體檢者(例如：公司員工體檢、勞工體檢、兵役體檢或供膳人員體檢等)，因體檢項目與教育部要求項目不完全符合，故不得以該報告直接繳交。

The University may also accept health examination reports issued within the past three months (e.g., employee health examinations, labor health examinations, military conscription examinations, or food handler health examinations). However, since such reports may not fully meet the examination items required by the Ministry of Education, they may not be submitted directly.

3.請先自行將已完成之檢查報告自行謄寫至本校體檢表，若有項目不齊全則需前往醫院補驗未檢項目，已完成之檢查報告請複印1份與體檢表一併繳交。

Students must first transcribe the completed results onto the NCNU Health Examination Form. If any required items are missing, students must go to a hospital to complete the missing examinations. A photocopy of the original report must be submitted together with the NCNU Health Examination Form.

4.若檢查項目不完整，則需補齊缺漏項目，否則視同未繳交。

If any required examination items are incomplete, the missing items must be supplemented; otherwise, the report will be deemed not submitted.

5.為有效防治校園傳染病及健康自我管理，並達到疾病早期發現、早期治療之目的，每位同學皆有進行健康檢查之責任；若未能於時間內(115年9月30日)繳交體檢報告者，依規定不能完成註冊程序，並需依本校相關規定辦理。

To effectively prevent communicable diseases on campus, promote self-health management, and achieve early detection and treatment of diseases, every student is responsible for completing the health examination. Students who fail to submit the health examination report by September 30, 2026 will not be able to complete the registration process and will be handled in accordance with the University’s relevant regulations.

6.居留健檢主要為傳染病篩檢，學生健檢則為常規檢查；兩者檢查項目，僅胸部X光檢查一項相同，其餘項目均不同，因而外籍生或僑生須接受2次健檢。
★ 提醒：僑生、外籍生及陸港澳生－除了入境時申請居留證所做的健康檢查之外，仍須一同做新生健康檢查。

The health check-up for residence focuses primarily on screening for infectious diseases, whereas the new student health check-up consists of routine examinations. The only shared component between the two is the chest X-ray; all other items differ. Consequently, foreign students and overseas compatriot students are required to undergo both health check-ups.★ Note: Overseas compatriot students, foreign students, and students from Mainland China, Hong Kong, and Macau must undergo the new student health check-up in addition to the health check-up performed when applying for a residence permit upon entry.

115學年度新生入學健康檢查常見問與答

Q1.學生生病或意外事件怎麼辦？

A1.1、校內設立臺中榮總埔里分院附設暨大門診，由家醫科、內科、精神科、中醫科之專科醫師與藥師駐診服務，並提供新生健康檢查異常追踪複檢服務。服務時間為週一至週五上午9:00~11:30；下午2:00~4:30，看診免收掛號費，只需付部分負擔，就診師生請於各時段看診結束前30分鐘至診間完成報到手續。

2、本校24小時值勤的單位有：駐衛警、校安中心及宿舍區的當日值勤輔導員。如有突發狀況（例如：住院、車禍、緊急送醫等），急需協助可以聯絡他們；因此，也請家長提醒學生注意緊急聯絡電話(049-2910000或學安生輔中心0932576184)，以備不時之需的良好習慣。

Q2.一定要做新生入學體檢嗎?

A2.1.依據教育部頒訂之學校衛生法第8條：學校應建立學生健康管理制度，定期辦理學生健康檢查」及學生健康檢查實施辦法第4條：「學校辦理新生入學時，應進行學生健康基本資料調查，並做成紀錄」。

2.本校依據學生衛生法、學生健康檢查實施辦法及學生健康檢查及疾病防制要點，規定全校新生(含非本校轉入之轉學生、復學生、境外生)一律須體檢，含在職專班及碩、博士班生，大學部直升研究所均應自費接受健康檢查。

3.健康是基本人權，學校為公共場所，有保護及增進學生健康的責任，健康檢查是學生、家長、學校了解學生健康的基本資訊來源，且為規劃、參與健康活動的依據。為維護學生健康，早期發現早期治療，及預防傳染病發生，本校新生入學時依上述規定應完成健康檢查。

Q3.是全省各家醫院都適用本校學生健康檢查團體優惠價（1100元）嗎？

A3.1.本校合約醫院為台中榮總埔里分院學生健康檢查團體優價1100元(049-2990833轉3861)，地址:南投縣埔里鎮榮光路1號;體檢時間:週一至週五08:30-11:00、13:30-16:00;國定假日休診。

2.埔里基督教醫院學生健康檢查為1200元(049-2912151轉3343)，地址:南投縣埔里鎮鐵山路1號；報告自取，體檢時間:週一至週五上午8:00~11:30；週二至週四下午1:30~4:30;國定假日休診。

3.本校檢查項目依教育部頒定學生健康檢查基準表，相關法令規章請參考教育部大專校院學生健康檢查工作手冊。

4.依據醫療法及醫療機構設置標準規定，醫院、診所、衛生所等機構屬前開規定所稱之醫療機構，學生健康檢查不包括醫事檢驗所。

Q4.已經參加過其他健康檢查者，是否仍須繳交本校體檢表?

A4.1、入學最近3個月曾接受其他體檢者，（例如：公司員工體檢、勞工體檢、兵役體檢或供膳人員體檢等），因體檢項目與教育部要求項目相差甚遠，故不得以該報告直接繳交。（超過6個月體檢報告，必須重新完成新生健康檢查）

2、健康檢查日期為115年6月1日以後，請將已完成之檢查報告自行謄寫至本校體檢表，若有項目不齊全則需補做檢查逐一完成。已完成之檢查報告請複印一份隨本校體檢表一併繳交。(以最近3個月內健康檢查，與本校體檢表相同者，方得免重複受檢)

3、體檢項目缺漏甚多時，建議檢重新檢查費用較優惠。

Q5.外國學生來台前已經做過「居留或定居健康檢查項目表」，請問是否可以取代新生健康檢查資料？

A5.居留健檢主要為傳染病篩檢，學生健檢則為常規檢查；兩者檢查項目，僅胸部X光檢查一項相同，其餘項目均不同，因而外籍生或僑生須接受2次健檢。
★ 提醒：僑生、外籍生及陸港澳生－除了入境時申請居留證所做的健康檢查之外，仍須一同做新生健康檢查。

Q6.請問我是新生休學、保留學籍者，那我還需要作體檢嗎？

A6：不用的，休學期間免體檢，請於復學前完成健康檢查繳回學務處衛保組。

Q7：我是轉學生，還要作新生健康檢查嗎？

A7： 本校轉學生得免做健檢，非本校轉入之轉學生，請依據學校衛生法第8條規定入學時，繳交最近3個月內的健康檢查結果報告，若為2025年6月1日以前體檢報告，必須重新完成新生健康檢查。

Q8：自行在校外公、私立醫院完成新生健康檢查要注意那些事項？

A8：1.請於開學繳交已完成的學生健康檢查表。

2.受檢前請事先詳填本校健康檢查資料卡正面個人資料（請貼好1吋照片，正面請完成所有資料及背面註明姓名、學號、系所）。健檢時請將學生健康檢查表帶至健檢單位，可避免有漏檢的項目發生。

3.至健檢單位領取健檢資料時，請務必核對是否有漏檢的項目，以免日後進行補檢;詢問醫院是否有牙科檢查避免缺漏該項目，若有缺漏時可自行到牙科診所補檢。

4.醫院體檢報告通常需要7-21工作天，還需再前往領取後，繳交或郵寄衛生保健組，請務必確認資料卡正面的問卷調查每一題都有勾選，避免退件郵件往返。

5.體檢表內項目可至醫院或診所(不含檢驗所)檢查，所有項目皆完成，醫院等級不同，收費標準不同，費用約1200~2000元不等。
Q9：參加學校的新生健檢，報告時麼時候才會出來？怎麼領取？

A9：目前採紙本報告，健檢報告約檢查後30天（不含國定假日、颱風假等），衛保組將於10月與台中榮總埔里分院合辦「新生體檢報告表說明會」，統一至系上說明並發放健康檢查表，如對報告有問題，皆可於說明會結束後至暨大門診詢問醫師。

★★檢查報告請妥善保存★★請自行妥善保管體檢報告，健康中心無法補發；因應個資法，若須申請報告請自行到醫院申請。

Q10：新生健康檢查項目若出現異常要怎麼辦?

A10：可參照健檢報告中''醫師總評''，若有建議至門診追蹤檢查，可持檢查報告至暨大門診或其他醫療院所就診。

Q11:我覺得自己生病了，可不可以叫救護車來接我呢？

A11:我們希望將緊急醫療資源保留給有生命危險的病人（例如：急症發作、臉色蒼白或臉色發黑、出冷汗、呼吸急促、氣喘發作、心臟病發、顛癇發作、昏倒、休克、嚴重疼痛、嚴重外傷、骨折、大出血等狀況)，除通知衛保組立即派醫護人員前往處理，同時請立刻打119 叫救護車儘速送至醫院急救，以免延誤搶救生命的黃金時機。若您的狀況並非危急且可以自行（或透過朋友、師長的幫助）就醫，希望您儘量將救護車資源保留給狀況危急而分秒必爭的病人。

Q12:胃痛、生理痛或其他身體不舒服，可以到衛保組拿成藥吃嗎?

A12:衛保組傷病處理僅止於簡易救護技術操作，依據醫療法不得執行醫療行為。依據：「藥事法施行細則」第3條，醫師處方藥物須由醫師處方或限由醫師使用。「護理人員法」第24條，護理人員業務執行醫療輔助行為(包含輔助藥物之投與)，應依規定由醫師開立處方藥物。依上述法規說明:醫療處方藥物或輔助行為應由暨大門診醫師處方指示下執行。
