Notice of Health Examination for Newly Enrolled Students

The physical examination for first-year undergraduate students will be conducted collectively on September 2, 2026. New master’s and doctoral students, in-service program students, and transfer students are required to complete the examination on their own at a hospital.

To strengthen student health management, facilitate early detection and treatment of health problems, and prevent communicable diseases on campus, all newly enrolled students of National Chi Nan University are required to complete and submit a health examination report upon enrollment in accordance with relevant regulations.

1. Purpose

To effectively control communicable diseases on campus and implement student health management, all new students are required to submit a health examination report.

2. Legal Basis

This requirement is based on the following regulations:

• Article 8 of the School Health Act:
  “Schools shall establish a student health management system and conduct regular student health examinations.”
• Article 4 of the Regulations Governing Student Health Examinations:
  “Schools shall conduct a basic health information survey of newly enrolled students and keep records thereof.”

Accordingly, all new students are required to complete the health examination upon enrollment in order to safeguard their health and prevent communicable diseases.

3. Applicable Students

• First-year undergraduate students: required to attend the on-campus health examination on September 2, 2026 (morning).
• New master’s students, doctoral students, in-service program students, transfer students, returning students, and undergraduate students directly admitted to graduate programs: required to complete the health examination independently at a hospital.

6. Examination Fee

To ensure the quality of the health examination and reduce the inconvenience and cost of individual hospital visits, the examination fee is offered at a group discount rate of NT$1,100.

7. Examination Items

The health examination includes a total of 39 items, including:

 Important Notes

8.1 First-Year Undergraduate Students Attending the On-Campus Examination on September 2, 2026

1. Please complete both sides of the health examination form in advance and bring the following on the examination day:
   • One recent photo (1-inch or 2-inch)
   • National ID Card or National Health Insurance Card
   • Foreign students: Passport or Alien Resident Certificate (ARC)
2. Please complete the Health Information Card through the University Administrative System:
   https://sso.ncnu.edu.tw/login
   Path: Log in to the New Generation Administrative System → Personal Information → Fill out the Health Information Card
3. Please note the following before the examination:
   • Avoid staying up late for three days prior to the examination
   • Fast for 8 hours before the examination
   • Wear clothing without buttons, metal parts, or sequins
   • Students who may be pregnant must not undergo the chest X-ray examination
4. Urine test:
   Please collect a midstream urine sample in a plastic cup and transfer it into the urine test tube. Students who are menstruating are still required to submit a urine sample; please inform the staff when submitting the specimen so that it can be noted.
5. Vision test:
   Students who wear glasses should bring them for the measurement of corrected vision.

8.2 Master’s/Doctoral Students, In-Service Program Students, Returning Students, and Transfer Students from Outside NCNU

1. Students may bring the NCNU Health Examination Form to a public or private hospital for the examination. Fees will be charged according to the standards of the respective hospital. After the examination, the completed form must be submitted to the Student Health Services Section, Office of Student Affairs.
2. The University may also accept health examination reports issued within the past three months (for example, employee health examinations, labor health examinations, military conscription examinations, or food handler health examinations). However, because such reports may not fully satisfy the examination items required by the Ministry of Education, they may not be submitted directly.
3. Students using an existing health examination report must:
   • Transcribe the completed results onto the NCNU Health Examination Form
   • Complete any missing examination items at a hospital
   • Submit a photocopy of the original report together with the NCNU Health Examination Form
4. If any required examination items are incomplete, the missing items must be supplemented; otherwise, the report will be deemed not submitted.
5. Students who fail to submit the health examination report by September 30, 2026 will not be able to complete the registration process and will be handled in accordance with the University’s relevant regulations.

4.Expenses

Under the co-payment policy of Taiwan's National Health Insurance, all patients will pay a percentage of their medical fees unless the treatment is being provided as preventative care, or the treatment is in relation to a catastrophic disease. In addition to the co-pay waiver on specific types of treatment, the government has enacted legislation that reduces the amount contributed to treatment by specific individuals, namely veterans and low-income families.

5.Hours & Location

The Campus Health Clinic follows the academic calendar. We are open Monday through Friday from 9 AM to 12 AM; 2 PM to 5 PM. No services during Winter Break and Summer Break.

NCNU Clinic Instructions and Procedures

Medical Process:

Registration → Consultation → Pricing → Medication Pickup

1.Registration:

New patients must fill out a first-time patient form.

2.Consultation:

(1) Bring your health insurance card for consultation.

(2) Bring your residence permit or passport for self-paid consultation.

3. Pricing:

(1) After consultation, go to the pricing window to pay.

(2) After payment, proceed to the pharmacy to pick up the medication.

4.Notes:

(1) Clinic Fees: No registration fee for those with a health insurance card; only a partial payment is required.

(2) Please complete registration procedures in the clinic 30 minutes before the end of each session.

(Morning session: before 11:30 am / Afternoon session: before 4:30 pm)

(3) Outdoor consultation and medication pickup services are provided for confirmed cases.

(4) Afternoon sessions offer medication exchange services and provide ointments for external injuries.

(5) International students paying for a consultation can get a diagnosis certificate and attach the original receipt for insurance claims. The application fee for a diagnosis certificate is NT$100.
