為協助疾病或遭遇意外事故之學生，並發揮社會救助之功能，特依據大學法第34條規定，凡具有本校學籍者(含實習教師)，均得參加學生團體保險。

一、每學期應主動繳交學生團體保險費，以維護自身權益，逾期未繳者，需自負責任。
保險日期：第一學期為8月1日至隔年1月31日，第二學期為2月1日至7月31日止。

二、有關於申請學生平安保險說明如下：

(一)更多資訊請至學生平安保險專區https://reurl.cc/ad5AGX查詢。

(二)填妥保險金申請書並檢附應備文件，再繳交或寄回學校衛保組核章及蓋關防。

(三)學生平安保險理賠給付範圍：意外事故急門診、住院可申請理賠。

【疾病】需住院或手術才可申請理賠；疾病急門診不給付。
需「縫合」的牙科手術才能給付，若是於門診進行的「拔智齒」，則不在理賠範圍內。

(四）若同一事故傷害需繼續治療，則於治療完畢再開立診斷書並請醫師註明回診次數。若治療一次，即可則當次就開立診斷書，以免日後需再掛號開立診斷書。

(五）若於不同醫療院所治療，需分別開立就診醫院之診斷書。 
(六）各項文件齊全，經核賠約30日可獲理賠。

三、申請學生團體保險需附文件如下：

(一)診斷證明書或住院証明(正本)(骨折須附X光片)

(二)醫療費用收據(若為副本或影本者必須『加蓋醫院鋼印或院章』)

(三)保險金申請書(自115年２月1日起一律使用『新版保險金申請書11501版』;需簽名、蓋章及填寫聯絡電話、聯絡地址)

(四)學生證正反面影印於同一張A4紙

(五)學生存摺影印本

(六)居留證正反影印本(境外生)

(七)道路交通事故當事人登記聯單

(八)初次罹癌，需檢附病理切片報告

四、保險理賠申請，學校僅代為收件，不予審查資料。審查部份屬保險公司人員專業處理之項目。

Student Safety Insurance

1.All NCNU students are covered by Student Group Insurance after paying the insurance fee each semester.

2.Insurance Period:Semester 1: Aug.1–Jan.31;Semester2:Feb.1–Jul.31

3.Please Remember:You must pay the insurance fee every semester.If you do not pay on time, your insurance rights may be affected.

4.If you are injured in an accident:You can apply for reimbursement for:emergency treatment;outpatient treatment;hospitalization

5.If you are sick:You can apply only if: you are hospitalized, or  you have surgery

6.Not Covered:regular outpatient visits for illness;outpatient wisdom tooth extraction

7.Dental Treatment:Dental claims are allowed only if sutures are required.

How to Apply:

1.Fill out the insurance claim form.

2.Prepare the required documents.

3.Send or submit them to the Health Services Section at NCNU for verification.

Documents You Need:

1.diagnosis certificate or hospitalization certificate(original)X-ray images(for fractures)

2.medical receipts.

3.if you submit copies, they must have the hospital official stamp.

4.insurance claim formfrom Feb. 1, 2026, only Version 11501 is accepted.

5.copy of your student ID (front and back on one page).

6.copy of your bank account/passbook.

7.copy of your ARC (front and back, for international students).

8.traffic accident report (if related to a road accident).

9.pathology report (for first-time cancer diagnosis).

Important Tips:

1.If you need follow-up treatment for the same injury, apply after the treatment is finished.

2.Ask the doctor to write the number of follow-up visits on the diagnosis certificate.

3.If you visit different hospitals or clinics, get a diagnosis certificate from each one.

4.It usually takes about 30 days to receive payment after all documents are complete.
